Regional Coordinating Centers


The Commonwealth Fund, Qualis Health and the MacColl Center for Health Care Innovation at the Group Health Research Institute launched an initiative to help primary care safety net sites (practices that serve Medicaid and uninsured patients) become high-performing patient-centered medical homes.

Five Regional Coordinating Centers were selected to participate in the demonstration project, and each Center partnered with 10 to 15 safety net clinics in their state. These collaboratives received technical assistance on practice re-design topics such as enhanced access, care coordination and patient experience.

The Centers also received funding to support a Medical Home Facilitator to lead clinic-based quality improvement projects and other activities. The work of the Regional Coordinating Centers began in April 2009. Technical assistance concluded in April 2013. An evaluation of the Initiative is now underway.
