Implementation Guides are available for each of the eight Change Concepts for Practice Transformation. The partner sites and Regional Coordinating Centers that participated in the SNMHI were members of a learning community working toward the shared goal of PCMH transformation. This learning community produced and tested ideas and actions for change, which informed the SNMHI Implementation Guide Series and other resources. We gratefully acknowledge the contributions of partner sites and Regional Coordinating Centers, and the knowledge and experience of the many organizations that partnered to support their efforts.
